Newport Beach OB/GYN Medical Group, Inc. Contents For Patients Medical Records

 

You are entitled to all your medical records at Newport Beach OB/GYN Medical Group, Inc.  These may be requested for other physicians or for yourself at any time.  You will need to sign a medical records release authorization form that you can mail, fax or drop off at our office.  Please be aware that a $15 processing fee plus $0.25/page will need to be collected before your records are released.  We strive to have them ready in the first few days of your request, but at times may take up to two weeks.
